Plan names vary by state: Silver Simple Women's Health with Menopause Benefits and Silver Simple Women's Health with Menopause Benefits Guided Care.

This plan is most suitable for women, typically age 45+, who are experiencing, or have experienced, natural or forced menopause, and their families.

- How will Oscar know if I completed the actions for the rewards and incentives?
Once you complete an activity, the claim from your visit (or prescription refill) will serve as verification of completion and you will be sent your gift card.
