Today, I’m going to share with you some SMART goals on how to better manage your chronic medical conditions. Some examples of chronic medical conditions include high blood pressure, diabetes, and high cholesterol. It’s important to make sure that you are getting good control of these conditions as they can lead to heart attack, stroke, or even death.
A quick refresher on SMART goals. SMART goals are designed to ensure that your goals are realistic, achievable, and meaningful.
For example, a SMART goal for diabetes management would be something similar to:
“I will be mindful of how certain foods impact your blood glucose levels by keeping a food journal and adding to it daily before my next visit with a primary care provider or endocrinologist.”
